Health & Well-being Projects For Mind Body & Soul
We have been providing healthcare in the homeless community in London Borough of Redbridge since 2006, working in partnership with The Welcome Centre Ilford, and many others.
We have been instrumental in supporting people experiencing homelessness in accessing appropiate health care. Our Nurse Practitioner heads up a team of healthcare professionals including general and mental health nurses, chiropodist, counsellor, exercise coaches and massage therapist.
When the client has registered with the Centre we then undertake a full and thorough health needs assessment to ascertain needs, support clients to register with a local GP, monitor and support them to improve their health and well-being. We are then able to referon to secondry and tertiary care as neccessary and appropiate.
People who are homeless experience more health problems than the general population; they are also more likely to die earlier (Average age 42) and use A&E services more often and inappropiately.
Our service has the proven benefit of reducing risk to clients health.
We have been able to continually improve the service offered with grants, which in the past have included LB Redbridge , City Bridge Trust and others. Currently we are funded by Reaching Communites Big Lottery Fund and this continues until February 2020. To see our interim report please click here
